Dr. Monica Ott, the Regional Medical Director for Abode Care Partners, presented two topics at the 2025 Society for Post-Acute and Long-Term Care Medical Association (PALTmed) Annual Conference. 

Dr. Ott has been an active member in PALTmed for many years and currently serves as the state president council representative on the national board. She is a board-certified geriatrician and certified medical director with expertise in dementia, pressure ulcers, frailty, and chronic diseases.

Breakthrough or Bust? IV Vitamins, Amniotic Membranes, Urine PCR, & Pharmacogenomic Testing

IV vitamins, amniotic membranes for wound care, urine PCRs, and pharmacogenetic testing (antidepressants & antipsychotics) are some of the new tests and products being utilized in PALTC. The panel of presenters will include a wound care expert and long-term care medical directors.

Using a point counterpoint format, the speakers will discuss risks, benefits, efficacy, and indications for each test or therapy. The discussion will include utility of the information obtained through urine PCR and pharmacogenomic testing and evidence-based indications for IV vitamins and amniotic membranes.

Antithrombotic Therapy & Anticoagulation Stewardship: Implementing Best Practices in PALTC

This interdisciplinary session will outline indication and up to date recommendations for antithrombotic therapy in PALTC. We will introduce antithrombotic stewardship, featuring insights from a pharmacist with the Anticoagulation Forum, as a system level approach to safe and effective implementation of best anticoagulation practices.

This session will delve into multilevel leadership opportunities and roles for physicians, Advanced Practice Providers (APPs), pharmacists, and other healthcare professionals with a focus on safety and patient-centered care. Additionally we will showcase actionable steps in quality improvement initiatives spearheaded by Medical Directors, APPs, and CMOs, aiming to optimize antithrombotic practices across healthcare settings.

The PALTC conference provides the latest developments and tools available to professionals to improve the delivery of quality care in post-acute and long-term care medicine.
