Abode Care Partners Nurse Practitioner Brittany Clunie has been Working Smart and Doing Good in Louisville, KY.
Brittany shared how she has learned the importance of continued patient advocacy. One recent patient was a 95-year-old resident with chronic medical conditions who experienced frequent hospitalizations. In previous care-planning discussions with the patient and his family, they had been reluctant to move forward with the recommended palliative and hospice care.
After reviewing new developments in this individual’s health status, Brittany called each family member personally. She spoke with them about their loved one’s prognosis and how different interventions would impact him. They then revisited the care-planning conversation at the next in-person appointment. After this meeting, he was placed on palliative care services before passing away peacefully one month later, without pain and in the comfort of his room with his family at his side.
