The foundation supports patients and families with their hospice journey. While the Foundation does not provide health care services to patients, it does provide financial support to families in need of non-health care related assistance.

The BrightSpring Hospice Foundation is a 501(c)(3) not-for-profit organization that serves all eligible patients in need. As such, donations made may be tax-deductible as permitted by law. Consult your tax professional for more information.

We accept applications from hospice social workers, nurses, and directors who apply to us on behalf of their patients. Applications must include an invoice or statement from the provider of the service being requested to be considered. Please note, the BrightSpring Hospice Foundation will only submit payments to service providers and does not send payments directly to the individuals requesting funds. The Foundation does not provide reimbursement for payments already made or items purchased.

To apply for support, please complete a patient/family service request form.

All applications will be reviewed with care by our board of directors to ensure the requested support falls within the guidelines of the organization.
