The three teamed up for a patient who had just been discharged after treatment for congestive heart failure and an infection, at a moment when his longtime primary care provider had left the practice and a new one was not yet in place. He needed medication management, education, labs, and close monitoring right away, as he was at real risk of ending up back in the hospital. Through the partnership between the two teams, Melanie and Ashley were able to see him quickly, which cleared the way for Adoration’s home health team to admit him without delay.
That timing made the difference.
